a) Please read the instructions carefully before filling up this form. Form should be clearly typed. Incomplete form will not be attended.

b) Companies who wish to participate in GreenTech Malaysia procurement activities are required to register with GreenTech Malaysia. However, registration does not guarantee registered suppliers are automatically included in the procurement activities.

c) Companies must have valid certification with the Companies Commission of Malaysia (SSM), Ministry of Finance and/or Construction Industry Development Board (CIDB) and/or Contractors Service Centre (PKK).Eligibility for participation in GreenTech Malaysia procurement activities shall be based on the categories approved.

d) The processing fee chargeable is RM106.00 (inclusive SST) per submission (non-returnable) for new registration and renewal. Payment shall be made to Malaysian Green Technology Corporation by Telegraphic Transfer (TT) Bank Account No: CIMB Bank 80-0215780-8, Cheque or Bank Draft. e) There is no charge for updating supplier’s information. Any change to the company particulars must be informed in writing within thirty (30) days from the date of the change.

g) Processing of applications may take 1-2 week upon successful receipt of the application form. Successful applicants will be informed via official letter and will be issued a registration number that will be used as reference in any related procurement activities. Application status also can be checked via GreenTech Malaysia websites.

h) GreenTech Malaysia reserves the right to reject, suspend or terminate any application/registration without any notice and without assigning any reasons.
